Band Registration Packet, 2024-2025
- This year’s Registration Packet is now completely digital. However, we must produce hard copies of these forms to obtain the proper signatures.
- I encourage you to complete the packet, preview and save the PDF before submitting it, and print out a copy to be submitted by July 29th, 2024.

You will need your insurance information to complete the SCPS Medical & Travel Form. This form also needs to be notarized to be considered complete.

You will also need a method to pay the initial payment for band assessments and order marching band accessories. We can accept various payment methods, including credit cards, digital payments via mobile devices, checks, and cash. We can even take money orders at a later date.
