Registration Packet 2012-2013.  All the forms required to sign up as a Marching Ram. The medical form must be notarized.  Late registrants may be assigned as “alternates” for the half-time show.

Student and Parent Band Agreement Form

Student Account Transfer Request  Use this form to pay your band dues from the money you have raised. By sending in this form, the Treasurer will apply the money in your Student Account  to your Fair Share payment.

County’s form to drive students in your personal vehicle for school events. Required ahead of time, provides liability coverage.(Please use “print preview” before printing.You may need to print it out as a “full page photo.”)

Reimbursement/Check Requisition Form. Use this form to get reimbursed for expenses on behalf of the band. All purchases must be pre-authorized in order to be reimbursed.
